If your velocity is turned down to far, the balls will fall out of the barrel. If your velocity is turned up to high, the ball can deform as it's exiting the barrel and the non uniform ball will go diving to the side, or up or down.

It could also be a paint issue. I've seen some wild stuff with old paint.
